Passport Services –
The Perry Public Library is an official Passport Acceptance Facility for the U.S. Department of State. Please call for more information or to make an appointment.

We provide forms for passport renewals, however, the applicant is responsible for mailing the form themselves.
We do not offer photo services. Please click here to see store locations that offer passport photo services.

Monday – Thursday 9:00 am to 7:00 pm
Friday 9:00 am to 4:00 pm
Saturday 11:00 am to 2:00 pm
How to Apply
You can find full passport application details on the State Department website, travel.state.gov. You can also call the National Passport Information Center at 1.877.487.2778.
If you are renewing your passport, be sure to read over the requirements. The applicant is responsible for mailing Renewal Applications to the U.S. State Department.
If this is your first passport, you must apply in person. You need:
- A completed, unsigned application for passport (Form DS-11) filled out in black ink
- Proof of U.S. citizenship
- Current proof of identity
- One passport photo
- A check or money order for US Dept. of State (see below under “Fees”)
Additional documentation is required for minors under the age of 16.
Fees
A check or money order must be paid directly to the U.S. Department of State and an execution fee is paid directly to Perry Public Library. Please make sure you bring two forms of payment: a check or money order is required for the US Department of State fee. The Execution Fee of $35.00 per application paid to Perry Public Library can be cash, check, or credit card. For more information about fees please visit travel.state.gov.

Can I get a passport for my child?
For all children under age 16:
All applications for children under 16 require both parents or legal guardians to be present during the application process. Birth certificates must be submitted for all children to parental relationship (even if the child had a previous passport).
For all children age 16 to 17: the minor must appear in person.
Where can I get information about the status of my application?
Status information is available through the National Passport Center, 1-877-487-2778. The Library is unable to provide information or updates about your application.
Can I apply for a visa at the library?
Passport application acceptance agencies do not handle visas.
We DO NOT recommend using services or websites that promise online passport acceptance, renewals, or expedited services. If you are unsure of a service's legitimacy, please call us at 440-259-3300 or the Department of State at 1-877-487-2778.
Notary Services:
The Perry Public Library offers free limited Notary Public service during regular library hours.
Please call in advance to verify that a notary is available.
Guidelines:
- The person signing the document must appear in person.
- Please bring a valid, government-issued photo ID and unsigned documents to be notarized. Documents must be signed in the presence of the notary in order to be valid (ORC Section 147).
- Please complete all information above the signature line completely.
- Patrons must bring their own witnesses, if needed. The library does not provide witnesses and witnesses may not be solicited from staff or customers using the Library.
- Documents to be notarized must be in English.
- Documents to be notarized must contain a Notary Public jurat or acknowledgement.
- The library provides only basic Notary Public services.
The Library’s free notary service is intended for simple documents (auto titles, etc.) that do not require specialized expertise. Documents we may not be able to notarize:
- Notary protests
- Employment eligibility verification, Homeland Security I-9 Forms
- Documents written in any language other than English
- Auto titles without buyer address and name (state law requires this section be completed, regardless of circumstance). Seller must be present.
No electronic notarizations unless the Notary is commissioned by the State of Ohio specifically for electronic notarizations.
Documents we will not notarize:
- Wills
- Financial Power of Attorney documents, or any related documents
- Healthcare Power of Attorney documents, or any related documents
We reserve the right to refuse notarizing documents for any reason. We reserve the right to decline service in cases that raise any issue of authenticity, ambiguity, or doubt.
Note: In the State of Ohio, notaries cannot certify documents. We cannot certify copies of passports, driver’s licenses, birth certificates, marriages, death certificates, divorce, or naturalization certificates.
